Privacy Policy

Dusome Dental Hygiene protects all personal and health information in accordance with PHIPA (Ontario privacy law), CDHO Standards of Practice, and HIPAA-aligned security practices for electronic communication. We collect only the information necessary to provide safe dental hygiene services, including your contact details, health history, clinical records, and treatment information. This information is used only for your care, recordkeeping, billing, communication, and compliance with CDHO and legal requirements. Your information is stored securely using encrypted systems, password-protected devices, and restricted access. Records are kept for 10 years, or 10 years past a minor’s 18th birthday, as required by law. We may share your information only with your consent, or when legally required—such as with another health professional involved in your care, insurers if you choose to submit a claim, or regulatory bodies (e.g., CDHO) when mandated. You may request access to your records, ask for corrections, or withdraw consent (except where record retention laws apply). Telehealth, email, and mail-in services follow the same privacy and documentation standards. Cancellation Policy

Cancellation Policy

24-Hour Cancellation Policy To ensure fair scheduling and availability for all clients, a minimum of 24 hours’ notice is required to cancel or reschedule any appointment. If a cancellation or rescheduling request is made with less than 24 hours’ notice, or if the client does not attend the scheduled appointment (“no-show”), the full amount of the booked treatment will be charged to the client’s account. This policy helps protect appointment times, ensures clinic efficiency, and allows us to continue offering high-quality client care. This Cancellation Policy does not apply to Dental Hygiene Services for Hire; for this service two (2) weeks notice is required. Thank you for your understanding and cooperation.

Warranty & Shipping Policies (Remote)

1. All clinical services (telehealth, assessments, impression reviews, documentation, fabrication steps) are non-refundable. 2. Custom appliances (sports/mouth guards, whitening trays) cannot be returned or refunded due to health-care and IPAC regulations. 3. Fit Warranty: • 30-day fit warranty for guards and trays. • Remake only if a confirmed fabrication or material defect exists. 4. Exclusions: • Damage from heat, chewing, pets, misuse, poor impressions, dental changes, cosmetic changes, or loss. Fit cannot be guaranteed when impressions are taken by the client. 5. Self-Impression Kits: • Self-impression kits are non-refundable and cannot be returned once shipped due to health and safety regulations. 6. Shipping (Fully Remote Service): • Client pays all shipping for impressions, adjustments, remakes caused by client factors, replacement materials, or cosmetic changes. • Dusome Dental Hygiene pays shipping only if the issue is a confirmed clinic fabrication error. • Shipping fees are non-refundable. • Not responsible for courier delays or lost packages. * Clients are responsible for all shipping costs related to remakes or adjustments resulting from client-related factors, including improper impressions or incorrect information. 7. Durability Warranty (90 Days): • Confirmed manufacturing (fabrication or material) defect. 8. Client Responsibilities: • Clients are responsible for providing accurate personal and shipping information at the time of order. Clients must carefully follow all instructions provided for dental impressions. Improper, distorted, or incomplete impressions may affect the fit and effectiveness of the custom appliance and are not covered under warranty. Clients are responsible for storing appliances properly and protecting them from heat, pets, and misuse. Clients must report any concerns within applicable warranty timeframes. Dusome Dental Hygiene is not responsible for issues arising from incorrect information provided by the client or failure to follow provided instructions. 9. Compliance: • Policy aligns with CDHO Standards, Advertising and Records Regulations, IPAC requirements, and Ontario Consumer Protection Act (where applicable
